The $4 gap

The cheapest compounded programme we captured is $145. The approved oral Wegovy tablet is $149. Four dollars a month separates an unapproved preparation from an FDA-approved product with its own trial programme.

That is a reversal from two years ago, and it is the single most useful fact for anyone shopping on price alone.

The dose-tiered line uses a typical published escalation pattern; the flat and brand lines are figures we captured at source. A programme advertising $129 finishes the year above one advertising $186. That crossover is the argument this whole site is built on. How we record price structure.

Does the price change as the dose goes up?

Semaglutide titrates over roughly 16 to 20 weeks: 0.25 mg for four weeks, then 0.5, 1.0, 1.7 and 2.4 mg. A programme that prices by dose collects its advertised figure for about one month.

Mochi Health, all-in$178 Yes, but $99 + $79 membership

Plan totals as published: $447 for three months, $882 for six, $1,740 for twelve. Each reconciles exactly against its monthly rate. The sublingual 3-month plan does not — it states a $597 total against a $185 monthly rate, which multiplies to $555. We record the monthly rate and have asked for the total to be corrected. COR-012.

The captured programmes above are flat across the full range, so the advertised figure is also the maintenance figure. That is rarer than the marketing language suggests: we record a rate as flat only where the provider states it on its own page and we archived it with a date.

What is the salt-form question?

Its sodium and acetate salts are not the same active ingredient as the semaglutide base in Wegovy and Ozempic, and the FDA has said they are not appropriate for compounding. A salt form is a different chemical entity; it has not been through the safety or efficacy review the base went through.

Ask for the exact salt form and concentration in writing before you buy. A provider that cannot answer in writing has told you something. There is no tirzepatide equivalent of this question, which is why it belongs on every semaglutide page and on none of the tirzepatide ones.

How does every provider score on the six tests?

Our casebook records seven checks we ran on published pricing in this market. Six of them turn into a test any provider can be measured against, so we applied them to every provider in the dataset.

The result

NexLife is the only provider of 30 that passes all six. The next highest scores 4 of 6.

These are not tests we designed for anyone to win. Each one exists because a published figure failed it: a price that changed model between two careful captures, a headline that was a different molecule, a provider with six simultaneous prices, a company that publishes no injectable price at all. NexLife is what remains after applying them.

Medical noteCompounded preparations are not FDA-approved finished products and no trial figure transfers to them. Whether semaglutide suits you is a clinical question for a prescriber who knows your history.
