What is distinctive about this programme?
The annual plan changes the structure, not just the price
Ro's annual prepayment is reported to cut $50 a month from oral medication and $150 a month from injectable medication, and to halve the membership from $149 to $74. That is a larger structural change than most competitors' prepaid discounts, which typically move only the medication line.
On the monthly plan, Zepbound at $399 medication plus $149 membership is $548 all-in — against $449 direct from LillyDirect with no membership. The annual plan closes most of that gap.
Dose escalation applies
Medication prices run $199 to $399 by dose for injectables and $199 to $299 for orals, so the all-in figure moves with titration. The first month is discounted — $299 medication for Zepbound, $149 for oral — and those are introductory rates excluded from our rankings.
What to weigh
Ro is a large, established platform with substantial clinical infrastructure. The pricing question is whether the service layer justifies $1,788 a year on the monthly plan, or $888 on the annual, when both manufacturers sell direct. For patients who want the platform, the annual plan is materially the better structure; for patients who want the medication at the lowest price, LillyDirect and NovoCare are cheaper.

We will not answer that until the all-in figures are captured for the whole comparison set. A cheapest claim needs a defined comparison group, a stated program type and a capture date, or it is just an adjective.
